If you want to refer someone you know, please fill out the form below. Your referral will receive a choice of one of the following offerings.

• First 3 hours of home care services free
• COMPLIMENTARY care planning session with a care advisor to develop a customized plan and roadmap
• One month of payroll administration free of charge

Terms & Conditions

• The new referee must not be a current or past client of the company
• Referee can only be referred to the Company once
• The referral program is only applicable for home care services
• The referee (the new client) can redeem his/her incentive on the first bill.

Termination and Interpretation

We reserve the right to suspend or terminate our ‘Share the Care’ reward program or to change these the terms and conditions at any time and for any reason in our sole discretion, with or without notice to you. All decisions & interpretations of this program will be at our sole discretion.
